The undersigned hereby certifies that he/she is duly authorized to sign this application on behalf of Applicant/buyer.

The buyer agrees to the following:

• The buyer verifies that all information is correct. If any information is found by the seller to
  be false, the seller may without notice cancel any orders in house, or any deliveries in
  progress to the buyer.
• The seller may, at any time and without notice, cancel all credit availability to buyer and
  refuse to make any credit advances.
• The seller may use the above information with a bank or other kind of financial institution
  for the purpose of obtaining all personal and business financial information of any kind.
• The buyer will pay all costs and attorney’s fees incurred by seller in relation to the
  interpretation or enforcement of any or all of our obligations whether or not a suit is filed
• Upon Approval of this application, Buyer hereby agrees to 30-Day payment terms. All
  invoice date. All accounts not paid as agreed shall be subjected to a service charge of 11/2%
  per month. (annual percentage rate of 18%)
